1. If the cost charged for benefits increases, a change can be to:
- Make a corresponding increase
- Revoke the election
2. Change in Service Provider -
a. Terminating a provider and hiring a new provider, then a change is allowed to reflect the cost of the new provider
b. Terminating a provider because a relative becomes available to take care of the child at no charge, then an election can be cancelled
For voluntary removal from group plan.
There is a letter for an employee who is still working the required number of hours to be eligible for the group, are enrolled, but decide to remove themselves and/or dependents off a plan. An example includes a member moving to their spouses plan. This is a voluntary removal from the group plan and does not allow a member to continue the benefits through COBRA.
