When recommended by a health care professional for, or as a result of, a medical condition, the cost of installing equipment in the home (such as a ramp or wheelchair lift) is a qualified medical expense.

Submit evidence of medical necessity (e.g., prescription, doctor's note) with the request for reimbursement.

Note: If the capital expenditure is a permanent improvement that increases the value of the property, the excess value is not reimbursable.

During an FSA open enrollment meeting, don't forget to mention the rules of HSA distributions. Even if your company doesn't offer this, others do and an employees ability legally to elect the FSA and receive reimbursements is effected by their spouses HSA election as well (i.e. mostly likely they can't make a FSA election).

We do have plan document language to address this though.

A quick message about late COBRA payments...a Participant has 30 days from the 1st of the month to make their payment. If they don't pay on time, the administrator has the right to terminate the coverage to the last full month of payment.

Sometimes, since the COBRA Participant is usually on the group's bill and they are paying as billed, the Participant's premium is being paid so the Administrator could be lenient and not term, plus accept the "late" payment.

There are key items that must be included on your statements/invoices when requesting for reimbursement. If these items are not included on the documentation, we will require more documentation from you in order to finish processing the claim:

• Name of Patient
• Name of Provider (Doctor, Hospital, etc)
• Date of Service (service date must be within current plan year / not reimbursed based off payment date)
• Type of Service (Co-Pay, X-ray, Dental Cleaning, Rx, etc)
• Amount of Service (not necesarilly amount you paid)

Visa Slips and Cleared Checks are not adequate pieces of documentation.
